Intervention: Automated intervention including automatically generated, EHR-linked reminders mailed to patients when screening was due (n = 1173); automated intervention plus assisted care with telephone support from medical assistants to complete screening (n = 1161); automated intervention and assisted care plus navigated interventions provided by a registered nurse (n = 1174); or usual care (n = 1167). Usual care was provided in all groups.
